Company and the Website are operated in the United States. 11. NOTE ABOUT THE HEALTHCARE SERVICES

Company provides certain healthcare services in collaboration with healthcare provider(s) (the "Healthcare Services"). Personal information collected through the Healthcare Services are subject to the "Consent to Behavioral Health Collaborative Care Model" and supplemental documentation that may be provided to you (together, "Healthcare Disclosures"). In addition, as described in the Healthcare Disclosure, the following subsections (a)–(d) apply to Company's collection of personal information from patients under thirteen (13) years of age who wish to receive certain Healthcare Services online ("Child Patients") with verifiable parental consent.

a. Collection of Child Patient's Information Online. The Healthcare Services may include live sessions through online services. Although a Child Patient's parent or legal guardian ("Parent") will be the primary participant of the sessions on the Child Patient's behalf, the sessions may capture the Child Patient's image or voice ("Child Patient's Information") if the Parent allows the Child Patient to participate in the session.

b. Use or Disclosure of Child Patient's Information. Child Patient's Information may be used to create recordings or generate transcripts of live sessions on the online platform hosting Company's electronic health record system and its third-party technology partner(s). Child Patient's Information used to generate transcripts will be deleted promptly after any such transcripts are created. Session recordings containing Child Patient's Information will be deleted after six months. Child Patient's Information will not be disclosed to any other third party or made publicly available without your consent, unless otherwise required by law or compelled by a court order or subpoena.

c. Parental Rights. Pursuant to applicable law, Parents may have the right to (1) access the description of the types or categories of the Child Patient's Information Company collects from the Child Patient; (2) refuse to permit further use or future collection of the Child Patient's Information; and (3) delete the Child Patient's Information.
